Required Documents for Enrollment

Proof of Residency

This document shows the residential address (not mailing address), such as a mortgage statement, lease agreement with attached signature page, utility bill, or a letter from a government agency in the caretaker's name of the student enrolling. All documents must be dated within 60 days, and if a utility bill is submitted, it must also include the following: SERVICE ADDRESS, PROPERTY ADDRESS, SERVICE TO, or BILL TO.

Proof of Age

Student’s birth certificate, passport, or a student’s driver’s license/State ID. (Please note: A social security card is not a valid document and is not required for enrollment.)

Oregon Certificate of Immunization Status

Because we are a public school, Oregon requires that we collect immunization records. You will submit this form with an electronic or handwritten signature.

  • Vaccination Education Certificate: If your child(ren) is not vaccinated for non-medical reasons, you will need to submit a Vaccine Education Certificate in addition to the Immunization Form in order to enroll in an Oregon public school. For further instructions and to obtain a certificate, please visit the Oregon Health Authority Vaccine Exemption website.

Most Recent Academic Records

  • Elementary and Middle School students: Please submit their most recent report card.

  • High School students: Please submit a copy of their most recent unofficial transcript that includes all credits earned and a progress report for their current courses.

District Notice of Intent to Return

  • This is an ORCA specific form that our enrollment specialist will send to your student’s current school district for approval. You will download this document from the Registration Form.

Dental Screening & Vision Screening

  • Only required for students 7 years & younger. These forms do not need to be filled out by a doctor to be accepted.
